L'assistenza domiciliare integrata : un modello abbandonato per la long-term care

L'assistenza domiciliare integrata (Adi) è stata finanziata con consistenti risorse nel Pnrr con l'obiettivo di coprire il 10% degli anziani over 65. Nel 2024 i nuovi assistiti sono aumentati del 141% rispetto al 2019, contro il 26% della spesa Adi. I finanziamenti Pnrr sono ancora fermi nei bilanci regionali e il potenziamento dell'Adi è avvenuto aumentando le prese in carico a bassa complessità. In realtà, il servizio non garantisce un'assistenza continua e globale per i bisogni complessi della non autosufficienza, a causa della carente integrazione socio-sanitaria delle cure, mentre la riforma della Long-term Care è sostenuta da finanziamenti insufficienti. Considerando l'elevata variabilità dei costi unitari tra le regioni, appare urgente definire tariffe a livello nazionale basate sulla stratificazione del rischio dei pazienti e associate a standard di servizio, in modo da aumentare i livelli di complessità dell'assistenza domiciliare verso i quali le Regioni dovrebbero convergere. Integrated home health care has been financed with substantial resources in the Nrrp with the aim of covering 10% of the over-65s. In 2024, the number of new patients increased by 141% compared to 2019, while the expenditure grew by 26%. Nrrp funding are still frozen within budgets allocated to regions, and the target has been achieved by increasing the number of low-complexity cases treated. Actually, the service does not ensure comprehen-sive and continuous home care over time for patients who require it, owing the lack of so-cial and health care integration, while the Long-term Care reform suffers from insufficient funding. Considering the high variability of unit costs between Italian regions, there is an urge to adjust national tariffs based on patients' risk stratification and linked to service standards so as to increase the levels of complexity care towards which the Regions should converge.
